Before your procedure

If you are having a gastroscopy, colonoscopy, or flexible sigmoidoscopy, you will need to complete an online admission form. Please select the location at which you are having your procedure from the list below and click on the “Admission Form” link. It is not necessary to complete an online admission form if you are having a capsule endoscopy.
